Printing HR Hire and Change Forms

About HR Forms

Departments use the following HR forms when working with State employees:

  • Hire/Rehire Form: Used to hire a new employee or rehire a terminated/retired employee into a vacant position number
  • Position Change Form: Used to process an existing employee’s move to a new position number (Action = Position Change)
  • Change Form: Used to process one of the following job change actions for an existing employee.
        
  • Additional Job
  • Concurrent Hire
  • Data Change
  • Earnings Distribution Change
  • Hire
  • Leave of Absence
  • Long Term Disability
  • Paid Leave of Absence
  • Pay Rate Change
  • Position Change
  • Rehire
  • Retirement
  • Return from Disability
  • Return from Leave
  • Short Term Disability
  • Termination

The forms are created as pdfs, and you'll use Adobe Acrobat Reader to view and print them.

Once printed, complete the forms by hand, send for the necessary approval signatures, and then forward them to Human Resources along with all of the necessary paperwork. If you are uncertain about how to complete the forms, contact the State Appointments staff in Human Resource Services.

Print the Hire/Rehire Form

Access Hire/Rehire by clicking on Main Menu > SBU > SBU Human Resources > XML Reports > Hire/Rehire Form

  1. Enter the 8-digit vacant Position Number that you will be hiring the new person into
  2. Click Search
  3. On the Hire/Rehire Form, enter the new employee's Empl ID if you know it
  4. Click the Print Hire/Rehire Form button
  5. Wait while PeopleSoft processes and the status icon status icon in the top right corner spins
  6. Adobe Acrobat will open in a new window with the Hire/Rehire Form displayed (if it does not display, then refer to the directions at the end of this document)
  7. While viewing the pdf of the form, hold down the control button on your keyboard and tap the p button (on your keyboard) to open the print options. Click Print
  8. Close the form window

Print the Change or Position Change Forms

Access Hire/Rehire by clicking on Main Menu > SBU > SBU Human Resources > XML Reports > Change Form (or Position Change Form)

  1. Enter the existing employee's Empl ID (Stony Brook ID) and click Search
  2. Click the Print Change Form or Print Position Change Form button
  3. Wait while PeopleSoft processes and the status icon status icon in the top right corner spins
  4. Adobe Acrobat will open in a new window with the Hire/Rehire Form displayed (if it does not display, then refer to the directions at the end of this document)
  5. While viewing the pdf of the form, hold down the control button on your keyboard and tap the p button (on your keyboard) to open the print options. Click Print
  6. Close the form window

If the HR Forms Do Not Display

The forms are displayed as pop-up messages in Chrome, Internet Explorer, and Firefox. If pop-ups are blocked in these browsers, then the form will not automatically display when you click the Print button. Instead, the blocked pop-up icon in the address bar or warning message will indicate that the pop-up was blocked.

To be able to view the forms, always allow pop-ups* for the PeopleSoft website (psns.cc.stonybrook.edu).
*note: after following the instructions to always allow pop-ups for the site, you may need to click the print option again to view the pop-up form
