  1. Click the Survey Options button. Make sure you are on the "Survey" tab (underlined blue above).
  2. Check off Anonymize Response "Do NOT record any personal information and remove panel association (not recommended)" in the Survey Termination section of the popup.
  3. Click Save Changes when you are finished.
